The smtp mail queue couldn't open the mail socket and generated an error. Probably because of maintenance, a general error, or high mail traffic at that time.

The header errors are caused since php can't submit header info after any output is sent, and the mail error sent debug output, so subsequent header data generated the "Cannot modify header information" errors.

Since the mail was in a queue, it would have been delayed and then tried again. Eventually it would have succeeded and sent the PM through.
